It’s that time of year.  Reports are being released left and right after collecting data from hundreds of thousands physicians regarding their 2012 compensation and career details.  This post is served to give you an update on the numbers specific to Family Medicine doctors based off of a few big named surveys.

2013 Family Medicine Compensation Data

Mean / Average – $166,662
Median – $170,000
Lowest – $83,331
Highest – $400,000
Starting – $140,000

21% of physicians reported a decrease in income
47% of physicians reported income remained the same
32% of physicians reported an increase in income

Overall, male physicians earn 30% more than women.
In Family Medicine, males earn 17% more than women.

By geographic region
Northeast – $170,000
Northwest – $162,000
Central – $184,000
Southeast – $180,000
Southwest – $164,000
West – $180,000

Family Physicians employed at hospitals had the highest compensations followed by partners in a practice.  The least compensated family doctors work in academic settings.

Healthcare reform is impacting family doctors.  29% have joined or plan to join an Accountable Care Organization compared to last year’s 12%.  Some doctors are moving towards concierge medicine (3%) while cash-only practices stayed the same (4%).

Career Satisfaction
28% of family doctors state they would choose the same specialty again.
62% would still choose medicine as a career.
20% would choose the same practice setting.

Overall, Family Medicine is a thriving specialty with the majority of incomes remaining the same or increasing slightly.  Satisfaction in the field is down however with only 28% of physicians stating they would choose the field again.  Last year, it was 32% but even so, the number overall is low compared to other specialties such as Radiology or Orthopedics.

I thought Medical Communications and Medical Humanities were pretty interesting.  Medical communications acknowledges that social media, blogging, and the like are an important part in Health and Patient Education.  It also trains you in medical writing and editing of top journals in primary care.  Medical Humanities touches upon the arts in medicine such as a way to heal through art therapy and to redesign medical spaces or applications.  Both of these incorporate a significant amount of time to learning, teaching, and keeping up with your clinical skills

Just like Internal medicine doctors are experts in adults, Pediatricians are experts in children.  Pediatrics is considered a primary care field and most patients will stick with you in to their late teens!  They are the first point of contact for many frightened parents with sick babies, children, and adolescents.

One of the downsides to this field and probably the saddest part is that eventually your patients will have to transition on to an adult doctor, whether family medicine or internal medicine.  But one of the cool things I’ve noticed rotating with my pediatric attendings is this smile on their face as they turn to me and say, “I took care of her as a child, and now I’m taking care of her children.”

That’s the lure of primary care, to be connected so personally with your patients as you both go through life.

The national average starting salary for pediatric doctors finishing residency is $135,000.  After you’ve been seasoned, the maximum salary we’ve seen is $271,000.

Most pediatricians make between $147,108 and $188,654 dollars.  As a pediatric hospitalist, you might be able to make more money while having a better lifestyle.  Hospitalists work in shifts instead of the typical 9 – 5 work day plus overnight call.

To become a pediatrician, you need to complete 3 years of residency in pediatrics.  That’s the norm for primary care specialties [ internal medicine and family medicine ].

Just like in internal medicine, pediatricians can do fellowships after residency in order to specialize in a particular body system.  Specializing will allow you to earn a higher salary.

Pediatrics is probably the most competitive of the 3 primary care specialties because they have the least amount of spots.  In the most recent match, there were only 209 residency programs that offered 2,428 spots.

Pediatrics is a very interesting field.  The amount of pathology that children can have is as vast as the adult world.  When I rotated as a resident in Salt Lake City and Minneapolis, I met many pediatric residents who had the biggest smiles on their faces from talking to their patients.  Their smiles were so big, you couldn’t help but smile too.  This makes me believe that this field is extremely rewarding for those who love children.  The salary, however, is just average.

Internal medicine deals with the prevention, diagnosis, and treatment of adult diseases.  Compared with family medicine, internists do not deliver babies or care for children.  Their focus is only on adults.

Internal medicine is one of the pillars for primary care.  They are often the first point of contact for many patients who have a health concern.  Many patients will stick with the same doctor for their life!

Internal medicine physicians have a flexibility unlike any other primary care field.  We’ll talk about that in the training section.

Just how much do internal medicine doctors make?  The national average for starting right out of residency is $156,000.  The median average salary is $182,005.  It’s a little strange that the average reporting starting salary for family physicians was a little bit higher.  But I’m sure there were some confounds to the study.

Majority of internal medicine physicians reported earning between $163,989 and $205,702.  The maximum salary reported was $238,000.  If you accept a hospitalist position, you can earn $250,000 and work half of the year!

Residencies in internal medicine are 3 years long which is the same for most of the other primary care specialties as well.  The benefit of internal medicine over the other primary care specialties is that there are much more fellowships for you to specialize in afterwards.  Do you see all of those categories on the right of the page there?  Most of those are accessible only to internal medicine physicians.

Nephrologists.  Gastroenterologists.  Infectious Disease.  Critical Care.  Cardiology.  Neurology.  Endocrinology.  Rheumatology.  Did I miss anything?  All of these specialties began with a 3 year residency in internal medicine.  The fellowships for these fields range from a couple years extra to many years extra!  See their respective posts for more details.

In the most recent match, 377 programs offered a total of 4,999 spots in internal medicine residencies.  That’s nearly twice as much opportunity than available family medicine spots.

Internal medicine doctors are specialists in adult medicine.  Their residency is the gateway to a multitude of fellowship opportunities that will let you specialize in a distinct field which will undoubtedly increase your salary.  If you’re a primary care enthusiast but not interested in delivering babies or taking care of children, take a look into this field and see if it’s the right fit for you.

Family medicine physicians are one of the three specialties of primary care.  They are often the first point of contact for many patients who feel sick or have medical questions.  Some patients stick with their family doctor for 30 years!

Family doctors have been called “the jack of all trades” because they are trained in delivering babies and taking care of babies, adolescents, teenagers, adults, and the elderly.  Family medicine is the comprehensive care for the full spectrum of different patients.

Just how much do family physicians make?  Let’s narrow our focus to the most common kinds of family doctors, which are the kinds who see an average of 20-30 patients per day for a hospital or their own clinic.  Let’s also narrow our focus to whether you are starting off in family medicine or have been seasoned over the years.  These are the salaries you can get as a family physician :

• Right out of residency – $161,000
• Maximum salary reported – $239,000

If you plan to practice obstetrics, then you can add an extra 10 – 20 thousand dollars.

Salary.com reports the national average to be $172,627.  As you can see from their bell curve, the majority of jobs have been reported to gross $152,858 to $198,882.

So those numbers are for the traditional family physician.  There are a few other kinds of practices that family doctors can do and that will earn a significantly more amount of money.  Have you heard of concierge medicine or direct-pay practices?  Family physicians can earn $300,000 to $500,000 or more.  I’ll talk about that in another post.

A family medicine residency is 3 years long.  If afterwards, you want to continue specializing, you can do a multitude of fellowships.  The most common fellowships for family medicine include obstetrics, women’s health, geriatrics, and sports medicine

Currently, there are 451 family medicine residency programs that offer a total of 2,730 spots.  With the growing shortage of primary care physicians, we are seeing more spots in family medicine open annually.  Just since last year there were 100 new spots!

Conclusion

I’m a family medicine doctor myself about to graduate from residency.  I found the national average for family physicians to be fairly accurate from a personal standpoint.  I’ll be making $160,000 to start.  Family medicine has been a rewarding experience for me and I’m looking forward to my career in it.  If you like talking to people and dealing with a little bit of everything, family medicine is for you.
