If you are a techie, this next interview in the MD Salaries Interview Series is a real treat.  Have you ever used MediMath on your iphone during those rounds at the hospital or to save yourself during a pimping session?

Well, Dr. Evan Schoenberg is the creator of that app plus 14 other medical apps and 3 remote control type apps.  He single handed wrote the code and launched those apps on iTunes as a part of his company, Regular Rate and Rhythm Software.  His company was formed in 2008 back when the Apple App store had recently opened.

And another thing, have you ever heard of Adium?  It’s one of the best chat clients (on a Mac) for combining all of your chats from MSN, AOL, Gmail, Yahoo, and Facebook into one window instead of having all those windows open and bogging down your computer.  Dr. Schoenberg has been a lead developer for Adium for the past 10 years!

These are products that I’ve been using daily for years on my Mac, iPhone, iPad, etc.  So it’s really awesome for me to talk to Evan about his life and bring you this interview.  When he’s not coding, he’s saving people’s eyesights on a daily basis as an ophthalmology resident.  Enjoy.

My grandfather was a huge tech enthusiast. He taught me to love computers – not just as black boxes for game diskettes but as programmable systems – and gave me my first, an IBM XT, when I was about 4 years old. I was programming BASIC on the Apple ][ machines and on my home PC in elementary school, then started playing with C in later years. Adium was what brought me into writing in Objective C, the language used by Mac and iOS devices; I was originally just an enthusiastic user, looking to make small changes to a young app so it would better suit my needs. Over the course of the first year working on the application, I first learned the language, then gained proficiency (mostly by repeatedly trying to accomplish specific goals until I found what worked). Soon I was invited to join Adam Iser, the application’s founding developer, as co-lead developer.

When Apple released the iPhone SDK, using the same language as Mac programming, it was a natural fit to move into writing iPhone apps.

There’s my grandfather again, at least originally! He was a pathologist and really convinced me that medicine was a field that could provide lifelong satisfaction. “The last great profession,” he called it. I think that may be overstating the case – there are a lot of grew professions – but it was a good sentiment and it got me started.

I gravitated toward ophthalmology for a number of reasons. I love the extent to which people value their vision; sometimes, people will do the Right Thing to protect their eyes even when they make poor choices about their heart, their lungs, and so on, because it has a very upfront importance to our day-to-day lives. I love the microsurgery of ophthalmology, a chance to handle fine details for a great result, and the OR in general. I enjoy all the challenges and triumphs of clinic, too, a space that for some surgeons is a bane. It’s a great fit.

It’s been a great adventure, and a successful one. For the first few months after MediMath, my first App Store app and one of the first clinical calculators on the App Store, was released, I remember saying to my wife every day after looking at sales revenue and my bulging inbox, “This might not last. We shouldn’t make any plans around this…” Four years and a dozen distinct apps under the RRR name later, some with basic/pro or free/paid pairs available in the App Store, I’ve seen growth I never would have imagined. I’ve also built apps on a contract or partnership basis when the project had a particular resonance, such as ACS Trials, an evidence-based cardiology app I built in collaboration with a group of cardiologists. It’s not Facebook or Instagram success, but for a solo designer/programmer working on projects I love, it’s perfect.

I have 1 employee who handles our email support, and an independent contractor who has collaborated on bringing ICD9 Consult and Medical Mnemonics to Android and is currently working on an Android version of Rowmote Pro, our most popular app which serves as a remote control, launcher, keyboard, and trackpad for Mac and jailbroken Apple TV. No venture backing – for where I am right now, with a dual-focus on ophthalmology and programming, I wouldn’t want to answer to anybody but myself. Who knows what the future will hold, though.

I’m continuing development on a number of the apps currently out for sale… and I’m debating whether to make the move to a more managerial role, with one or two hired programmers, but letting go is hard.

I’d rather not give specific numbers, but I can say that I paid off my medical school loans in their entirety during my intern year, and the business has continued to grow since then.

I love every part of operating, so I do plan to practice. Ideally, I will eventually have a practice setting that allows me to dedicate some time to patients and some time to business development outside of clinic and the OR… It’s almost unfortunate how much I like ophthalmology, because there’s such infinite potential in the world of software development!

I think we all know that.
But just how much more do they earn over, say, a lifetime?

Well, a nationwide study, conducted by the University of California Davis Health System sought to find an answer.  They noticed a gap of about $2.8 million dollars on average.  Yes, that’s right, PCPs make about 2.8 million dollars less than their Specialist counterparts.

But there is some wide variation.  For example, a medical oncologist earns $4,288,906 more than PCPs over their lifetime while some specialties only earn $761,000 more than PCPs.

Examples of Lifetime Earnings:

• Surgery: $4,588,249
• Internal medicine subspecialties and pediatric subspecialties: $4,100,183
• All other medical specialties: $3,761,930
• Primary care: $3,000,527

The study evaluated information from 6,000 physicians using their data from 2004-2005.  The numbers were also adjusted for certain demographics including where you practice, sex, board certification, US vs international student, etc.  In total, 41 medical specialties were reported in the study.

With student loans reaching $250k in addition to other personal financial matters, it’s hard not to consider salaries when choosing a medical specialty.  We continue to see medical students choosing specialist residencies more than primary care roles further exacerbating a shortage of PCPs in the nation.  But don’t be so hard on them, because sawing bones or touching the heart on a daily basis is extremely awesome and worthwhile to the patients that have a better quality of life because of it.

Pain is a complex beast.  People can have chronic pain from cancer, arthritis, injuries, or just plain back pain.  The Institute of Medicine issued a report stating that about 100 million Americans suffer from chronic pain.  That’s more than heart disease, cancer, and diabetes combined!

Pain management comprises of a multi-disciplinary approach to the treatment of chronic pain.  Meaning it’s usually a team of pain doctors, psychologists, and therapists all working together towards reducing the amount of pain a patient has.  There are 2 major ways to become a pain management doctor.  They are both high in salary but there is a significant difference.

Just how much do pain medicine doctors make?

A reported median is $340,506 with the most common ranging from $273,297 to $398,304.  These numbers align more with pain management doctors who do not practice anesthesiology.  Doctors who do practice pain medicine with anesthesiology report an average of $502,024 per year!  If you are just starting out, anesthesiology pain doctors might make $315,000.  The maximum reported salary is $651,000!

Here are a couple pain doctors and their salaries :

• Dr. Desmond Henry, anesthesiology and pain management, pediatrics – $558,250
• Dr. Dhanalakshmi Koyyalagunta, medical director, pain management – $364,400

Keep in mind the difference that there is a difference between doctors who incorporate anesthesiology into their practice and doctor’s that don’t.

To become a pain management doctor, you can go one of two ways:

• Anesthesiology –> Pain Medicine Fellowship
• Physical Medicine & Rehabilitation –> Pain Medicine Fellowship

With over 100 million Americans suffering from chronic pain, the call for Pain Management doctors is still strong.  It’s a field of medicine that can truly bring relief to many patients.  The salary and lifestyle are also pretty great.  If you like doing procedures in an office or practicing integrative medicine, this could be the field for you.

Baby boomers are defined as those born during the post World War II baby boom between 1946 and 1964.  2011 minus 1946 equals 65 years old.  We are at that time period where the baby boomers are retiring, when their bodies are needing medical care akin to their age level.

Geriatricians are specialists in diseases of the elderly.  76 million baby boomers are retiring within the next  18 years.  There is definitely a demand for these physicians.

Because geriatrics is essentially a focused part of primary care, the salary is similar to other primary care physicians.  The median expected salary is $171,323 with the majority making between $158,222 and $193,847.

There’s a concern that the salary may be less given that medicare’s reimbursement rate is so low.  It’s possible.  Though, there are some physicians who charge a retainer fee like the Concierge Medicine model.  This would give you a higher income.

In order to become a geriatrician, you can do a fellowship after a family medicine residency or an internal medicine residency.  This will make you eligible to sit for the geriatrics board exam.  If you pass, you’ll receive a Certificate of Added Qualification in Geriatrics.

Currently there are 105 geriatrics fellowships by way of internal medicine and 44 fellowships by way of family medicine.

With 76 million baby boomers retiring in the next 18 years, geriatricians will be in hot demand.  You don’t necessarily need to have the added fellowship to practice medicine for the elderly, but it does make you more knowledgeable and more credible.  If you’re interested in the diseases of the elderly, you should consider this position to see if it suits you.

Cancer is often approached by a multi-disciplinary team : medical oncologist, surgical oncologist, and radiation oncologist.  Radiation oncologists are different from radiologists and medical oncologists.  These are physicians trained in treating cancer with radiation.  Different residencies and skillsets needed.

Just how much do radiation oncologists make?

The median expected salary is $329,784 with the majority making between $255,706 and $408,520.  If you’re just starting out, you might make $241,000 to start but quickly rise to the median after a couple years.  The maximum reported salary is $787,000!

The total residency training required to become a radiation oncologist is 5 years.  The first year is usually a preliminary or transitional year in adult medicine followed by 4 years of training in radiation oncology.  There are a couple fellowships available for further experience in the field.

The field is competitive as there are only 87 residency programs offering about 200 spots.

Patients with cancer need compassionate and caring physicians helping them.  Radiation oncologists receive this training on top of their training in using radiation to treat cancer.  There’s a lot of physics involved but if you enjoy learning about it and helping others with some of the most devastating, life-altering diseases, you should consider this field.
